Scope: this is a combined family planning estimate. It models the 2026–27 family low-income threshold and aggregate family reduction. It does not allocate the final levy between spouses, model prescribed-person exemptions, part-year marriage, family agreements or every tax-return adjustment.

2026–27 threshold

$47,238 base family threshold

The standard family income threshold is $47,238, increased by $4,338 for each relevant dependant. For people entitled to the senior and pensioner tax offset threshold treatment, the base family threshold is $61,623.

Where family income is within the applicable threshold, no Medicare levy is payable under the family low-income rule.

Family levy and MLS are different

The family low-income reduction changes the ordinary Medicare levy. The Medicare levy surcharge is a separate additional charge for some higher-income people without appropriate private patient hospital cover.
